Patient Portal Access


You can now access your patient health care information online.  For added security, a 5-digit PIN is required to view your patient information.  If you do not have a PIN, click the "register" provided on the website to complete registration.  Here you can view your summary of office visits, list of current and new medications, vaccination records, tests results, and future appointment information.

We now have a new feature available via the patient portal that allows secure messaging between patients and providers.  Patients are able to log into the portal and send a message to the office staff or to the provider.  Requests for prescription refills can also be sent through the portal.  Additionally, patients may schedule appointments with our providers via our patient portal.
